ABSTRACT

Objective: To analyze the safety and efficacy of combined left atrial appendage (LAA) and patent foramen ovale (PFO) closure in adult atrial fibrillation (AF) patients complicating with PFO. Methods: This study is a retrospective and cross-sectional study. Seven patients with AF complicated with PFO diagnosed by transesophageal echocardiography (TEE) in Zhoupu Hospital Affiliated to Shanghai University of Medicine & Health Sciences from June 2017 to October 2020 were selected. Basic data such as age, gender and medical history were collected. The atrial septal defect or PFO occluder and LAA occluder were selected according to the size of PFO, the ostia width and depth of LAA. Four patients underwent left atrial appendage closure(LAAC) and PFO closure at the same time. PFO closure was performed during a one-stop procedure of cryoablation combined with LAAC in 2 patients. One patient underwent PFO closure at 10 weeks after one-stop procedure because of recurrent transient ischemic attack (TIA). All patients continued to take oral anticoagulants. TEE was repeated 8-12 weeks after intervention. In case of device related thrombus(DRT), TEE shall be rechecked 6 months after adjusting anticoagulant and antiplatelet drug treatment. Patients were follow-up at 1, 3, 6, 12, 24 months by telephone call, and the occurrence of cardio-cerebrovascular events was recorded. Results: Among the 7 patients with AF, 2 were male, aged (68.0±9.4) years, and 3 had a history of recurrent cerebral infarction and TIA. Average PFO diameter was (3.5±0.8)mm. Three patients were implanted with Watchman LAA occluder (30, 30, 33 mm) and atrial septal defect occluder (8, 9, 16 mm). 2 patients were implanted with LAmbre LAA occluder (34/38, 18/32 mm) and PFO occluder (PF1825, PF2525). 2 patients were implanted with LACbes LAA occluder (24, 28 mm) and PFO occluder (PF2525, PF1825) respectively. The patients were followed up for 12 (11, 24) months after operation. TEE reexamination showed that the position of LAA occluder and atrial septal defect occluder or PFO occluder was normal in all patients. DRT was detected in 1 patient, and anticoagulant therapy was adjusted in this patient. 6 months later, TEE showed that DRT disappeared. No cardiovascular and cerebrovascular events occurred in all patients with AF during follow-up. Conclusions: In AF patients complicated with PFO, LAAC combined with PFO closure may have good safety and effectiveness.

ABSTRACT

Gastric polyps are frequently occurring diseases of the digestive system, which are easy to recur. Some of the polyps are prone to canceration. Western medicine is currently under endoscope surgery. Although the curative effect is exact, the recurrence rate is high. Professor ZENG Bin-fang, after years of clinical research and experience, concludes that the basic pathogenesis of gastric polyps is deficiency of spleen and stomach, spleen qi deficiency and qi deficiency through the disease, and they are also important reasons for its recurrence. Accordingly, invigorating the spleen, invigorating qi and activating blood circulation, regulating qi and reducing phlegm dampness, and strengthening the body resistance to eliminate pathogenic factors are proposed to treat gastric polyps. In addition, the Yiqi Sanjie Decoction was created, with good clinical efficacy. But for gastric polyps of d> 0.5 cm, it is still recommended to take gastroscopic treatment before taking TCM treatment, which can shorten the course of TCM, relieve the physical and mental burden of patients, and prevent patients from interrupting treatment because they can not take TCM for a long time, to affect the curative efficacy.

ABSTRACT

AIM To prepare the thermosensitive intestinal gels of Houttuynia cordata Thunb volatile oils hydroxypropyl-β-cyclodextrin (HPCD) inclusion compound.METHODS For the gels prepared by cold dissolving method,poloxamer 407 consumption and poloxamer 188 consumption were taken as influencing factors,together with phase transition temperature as an evaluation index,central composite design-response surface method was applied to optimizing the formulation.With 2-undecanone as an index component,the gels' dissolution rate and in vitro release rate were investigated by non-membrane dissolution method and dialysis bag method respectively,whose stability was then evaluated by high temperature (40,60 ℃),low temperature (4 ℃),strong light [(4 500 ±500) 1x] and acceleration (three months) tests.RESULTS The optimal conditions were determined to be 20.61% for P407 consumption and 3.03% for P188 consumption,the phase transition temperature was 36.5 ℃.Within the time range of 30-150 min,the HPCD inclusion compound gels exhibited higher accumulative dissolution rate than the volatile oils gels,which tended to be consistent in 150-210 min,but the former exhibited higher accmulative release rate (0-50 h) than the latter all the time.The obtained gels showed good stability at low temperature,whose appearance,characteristic (except for high temperature) and pH were stable at high temperature,strong light and acceleration with obviously decreased 2-undecanone content.CONCLUSION The thermosensitive intestinal gels of Houttuynia cordata Thunb volatile oils HPCD inclusion compound should be stored at low temperature (4 ℃).

AIM To prepare nanostructured lipid carriers for volatile oils from Artemisiae argyi Folium.METHODS Heated melting-ultrasonic dispersion method was applied to preparing lipid carriers.Taking solid/liquid lipid ratio,amounts of lipid,emulsifier and volatile oils as influencing factors,and average paticle size as an evaluation index,the formulation was optimized by orthogonal test.With cineole,camphor and borneol as indices,GC-MS was adopted in the content determination of volatile oils.RESULTS The optimal formulation was determined to be 5 ∶ 5 for solid/liquid lipid ratio,1%,3% and 0.5% for amounts of lipid,emulsifier and volatile oils,respectively.The obtained clear and transparent lipid carriers demonstrated the average particle size of (72.33 ±1.93) nm,PDI of 0.273 ± 0.004 5,and Zeta potential of (-30.59 ± 1.42) mV,whose in vitro release rate was lower than that of raw medicine within 120 h,along with a higher stability under 4 ℃ than that under 25 ℃.The entrapment efficiencies of cineole,camphor and borneol were 87.49%,86.45% and 92.12% with the drug loadings of 8.25%,2.00% and 3.38%,respectively.CONCLUSION It is suggested that nanostructured lipid carriers for volatile oils from Artemisiae argyi Folium should be stored under 4 ℃ with the features of sustainedrelease and stable physicochemical properties.

ABSTRACT

Objective: To investigate the correlation of the expression of pokemon with p 14ARF and bcl-2 in non small cell lung cancer (NSCLC) tissues, and their values for determining the clinicopathological characteristics and prognosis of NSCLC. Methods: Immunohistochemical SP staining was used to detect the expressions of pokemon, p 14 ARF, and bcl-2 proteins in 62 NSCLC tumor tissues and 20 adjacent normal tissues. Their relationship with the pathological characteristics of NSCLC was analyzed. The correlation of abnormal expressions of pokemon and p 14 ARF and bcl-2 was analyzed. The effects of the expressions of the three proteins on the long-term prognosis of NSCLC patients were observed based on the follow-up data. Results: In normal tissues there was no pokemon expression but the positive rates of p 14 ARF and bcl-2 expression were 95.0% and 15.0%, respectively. In NSCLC tissues, the positive rate of pokemon, p 14 ARF,and bcl-2 protein were 72.6%, 66.1%, and 53.2%, respectively. The expression of pokemon had negative correlation with expression of p 14 ARF (r = - 0.287, P < 0.05), but had positive correlation with expression of bcl-2 (r = 0.293, P < 0.05). The expression of pokemon and p 14 ARF was associated with TNM staging (P < 0.05). However, the expression of bcl-2 correlated with pathological classification (P < 0.05). In pokemon and bcl-2 positive patients, the five-year survival rates were 10.95% and 13.74%, respectively, which were significantly lower than that in pokemon and bcl-2 negative patients (P < 0.05). In contrast, the five-year survival rate was 21.68% in patients with positive expression of p 14 ARF, which was significantly higher than that in patients with negative expression of p 14 ARF (P < 0.05). Conclusion: Pokemon, p 14 ARF, and bcl-2 are all expressed in NSCLC tissues. There is a negative correlation between the expression of pokemon and p 14 ARF and a positive correlation between the expression of pokemon and bcl-2, which has a certain clinical significance for evaluating the prognosis of NSCLC patients.

ABSTRACT

<p><b>BACKGROUND</b>Transcription factor Pokemon, a central regulation gene of the important tumor suppressor ARF gene, exerted its activity by acting upstream of many tumor-suppressing genes and proto-oncogenes. Its expression in non-small cell lung cancer (NSCLC) and its clinical significance remains unclear. The aim of this study was to investigate the expression of Pokemon in NSCLC and to explore its correlation with the clinical pathological characteristics and its influence on patients' prognosis.</p><p><b>METHODS</b>Fifty-five cases of NSCLC were involved in this study. The expression of Pokemon in the tumor tissue, the corresponding tumor adjacent tissue and the surrounding tissue was detected via reverse transcription-polymerase chain reaction (RT-PCR) and Western blotting, with the aim of investigating the correlation between the expression of Pokemon in tumor tissue of NSCLC and its clinical pathological characteristics. Moreover, a prognostic analysis was carried out based upon the immunohistochemical (IHC) detection of the expression of Pokemon gene in archival tumor specimens (5 years ago) of 62 cases of NSCLC.</p><p><b>RESULTS</b>Statistical significance of the expression of Pokemon mRNA and protein was determined in the tumor tissue, the tumor adjacent tissue and the surrounding tissue (P<0.05). The expression of Pokemon was determined not to be associated with the patients' sex, age, smoking condition, tumor differentiation degree, histology and lymph node metastasis condition. However, its relationship with TNM staging was established (P<0.05). Furthermore, it was shown that the survival rate of patients with negative Pokemon expression was significantly higher than that of those with positive Pokemon expression (P=0.004), therefore, the expression of Pokemon is believed to be an independent factor affecting prognosis (P=0.034).</p><p><b>CONCLUSION</b>Pokemon was over-expressed in NSCLC tissue and the expression of Pokemon might be of clinical significance in non-small cell lung cancer prognostic evaluation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of long-term administration of fluvastatin on improvement of ventricular remodeling of rats after myocardial infarction and its mechanism.</p><p><b>METHODS</b>Sprague-Dawley rats were subjected to ligation in anterior descending branch of coronary artery and treated with fluvastatin (20 mg.kg(-1) d(-1)) or distilled water for 8 weeks. Doppler echocardiography, hemodynamic study and cardiac histomorphometry were used to estimate the ventricular remodeling and cardiac function. Laser scanning confocal microscope was used to definite the distribution of superoxide anion (O(2)(*-)) and nitrogen monoxide. RT-PCR and immunohistochemistry were used to detect the expression of NOS2 and p22phox in mRNA and protein level. The level of lipid peroxidation, glutathione peroxidase, nitrogen monoxide and total cholesterol were detected too.</p><p><b>RESULTS</b>Administration of fluvastatin ameliorated left ventricular remodeling without affecting the infarct size [(40 +/- 6 vs 42 +/-5)%, P>0.05]. The level of left ventricular end-diastolic pressure [(18.24 +/-6.58 vs 10.74 +/-4.71) mmHg, P<0.05], right ventricular ameliorated relative weight [(0.92 +/-0.19 vs 0.71 +/-0.13) g/kg, P<0.05], the thickness of left ventricular posterior wall [(3.04 +/-0.28 vs 2.60 +/-0.36) mm, P<0.05] decreased after fluvastatin treatment. The left ventricular ejection fraction was not influenced, the relative lung weight and the left atrium diameter reduced [(5.79 +/-2.92 vs 3.69 +/-0.68) g/kg, (0.55 +/-0.12 vs 0.45 +/-0.04) mm, P<0.05]; the expressions of LPO in the plasma and myocardium [(8.64 +/-0.59 vs 7.71 +/-0.66) U/dl, P<0.05; (3.12 +/-0.38 vs 1.93 +/-0.40) ng/microg.pro, P<0.01] were reduced, and the overexpressed NO was inhibited [(436.87 +/-47.22 vs 313.78 +/-34.35) mg/dl, P<0.01], but the expression of GPx increased [(66.13 +/-8.31 vs 79.78 +/-2.38) mg/dl, P<0.01]. The expression of O(2)(*-) and the activity of NADPH oxidase subunit p22phox increased; NOS2 and its products NO were over-expressed too.</p><p><b>CONCLUSION</b>Ventricular remodeling and hemodynamics are improved profoundly in MI rats treated with fluvastatin. The effect of antioxidative stress of fluvastatin might be involved in the mechanism.</p>
